ABOUT OUR SPECIALTY

Our facial plastic surgeons include five of the most uniquely qualified facial plastic surgeons in Indiana and are among the only surgeons in Indiana who are fellows of the American Society of Ophthalmic Plastic and Reconstructive Surgery (ASOPRS).

Fellowship in this specialized and exclusive organization is restricted to ophthalmologists who have been board certified by the American Board of Ophthalmology and who have received extensive training and testing in plastic reconstructive surgery of the eyes and face. This highly specialized certification assures you of the very best care available, offered by surgeons who set the the standards for excellence in aesthetic eye and facial surgery.

 

FEES, BILLING AND INSURANCE

We participate in most health plans. Medicare, Aetna, Anthem, Cigna, Blue Cross and Blue Shield, Sagamore, United Healthcare are only a few of the many plans with which we participate.

We also provide services for personal injury cases, such as: motor vehicle accidents (MVA), workman's comp and litigation.

APPOINTMENTS

Your first visit is for consultation only. Any surgeries will be scheduled for a later date.

Our office hours are Monday through Friday 8:00 AM to 5:00 PM (Eastern Standard Time). An after- hours answering service is available all other times for emergency situations. Our office manager may be contacted by using this link.

The following paperwork should be completed prior to your appointment and brought to the office with you along with a current government issued photo ID and any insurance cards.

 

Patient Registration Information

So that patients experience a rapid check-in on the day of service, the practice makes available patient pre-arrival documents as a PDF download. These documents include vital information regarding the federal Health Insurance Portability and Accountability Act, commonly called HIPPA.

To access your record you will need to sign up for an account at https://mdi.intellechartportal.com

To get the identity code required you must contact the office at 317-817-1976 and ask to speak with the office contact for portal access.
